Membership Reimbursement & Superbills

At Concierge Pediatrics, our membership model allows us to focus on what matters most – your family’s health. Although we do not participate with insurers, we understand that some families may want to explore whether their insurance company will reimburse a portion of their membership or visit costs. Superbills can be a helpful option for people who have out-of-network insurance coverage. While Concierge Pediatrics is not involved in the insurance reimbursement process, we’re happy to provide a superbill for fully paid memberships upon request so you can submit it to your insurance carrier for potential reimbursement.

Here’s how it works:

How to Submit a Superbill for Potential Insurance Reimbursement

Step 1: Request Your Superbill from Concierge Pediatrics & Sign Forms

  • A superbill is a detailed receipt showing the services your child received, the associated diagnosis codes, and the amount paid. A superbill is not the same thing as an invoice for membership.
  • You can request a superbill for any eligible visit immediately following your visit by asking our team member at the front desk. Superbills can only be provided from your Concierge Pediatrics office team. This cannot be provided by our membership director nor billing team.
  • Before receiving your superbill, our team will provide you with a form to sign.
  • Upon signing the form, we’ll provide you with a PDF that’s ready to submit.

Step 2: Submit the Superbill to Your Insurance Carrier

  • Visit your insurance company’s website or mobile app.
  • Search for “Submit a Claim” or “Out-of-Network Reimbursement.”
  • Upload your superbill and complete any required forms.
  • If you’re unsure how to proceed, contact your insurance provider’s member services line for step-by-step help—every plan is different.

Important Things to Note 

  • Reimbursement is not guaranteed. It depends on your individual plan’s benefits and how they choose to proceed.
  • Superbills are only available upon request and must be submitted to the insurance company by you, the member. Superbills are not submitted by our office.
  • Vaccines and telemedicine visits are unable to be included on a superbill.
  • Superbills can only be provided for payments to date. If a membership is not paid in full and monthly or quarterly payments are being made, a superbill can only be provided for the amount paid to date.
  • We do not bill insurance or communicate with insurance companies. The reimbursement process is entirely between you and your insurance provider.
